Despite existing medical infrastructure, the final stage of care between hospital and home often remains unmet. This results in untreated pain, patients discontinuing treatment and avoidable suffering, particularly amongst bedridden patients at the end of life.

The project enables prompt and continuous access to essential palliative care medicines in the home environment. To this end, selected and trained community carers are deployed under close professional supervision to safely deliver pre-packaged, patient-specific medicines to patients’ homes.

Through the use of permanently assigned community carers, written records of medication handover and regular home visits by nursing staff (nurses with palliative care training), it is ensured that patients receive their prescribed pain and symptom relief medication on time and correctly. This helps to prevent pain crises, reduces the number of treatment interruptions and enables the seriously ill to spend the final phase of their lives with less suffering and in dignity.

The results of the pilot project phase, which has now been completed, show that the project is effective. Among other things, 92 per cent of patients receiving care report improved access to medication, whilst 90 per cent of their relatives feel a noticeable reduction in the burden on them. It is therefore all the more gratifying that this important project will receive support from the ‘24 Good Deeds’ Advent calendar during Advent 2026!
